COVID Vaccines
FPC COVID-19 VACCINE UPDATE: 03/26/2021
Six FPC offices have been selected by the PA Department of Health to provide COVID-19 vaccines.
You may contact the location of your choice to schedule or to be added to a waiting list.

FPC Patients - if you do receive a COVID-19 vaccine at a location outside of Family Practice Center, please provide us with a copy of your vaccination card so that we can update your medical record!

Please note: If you are having a medical emergency, call 911 immediately.
Contact your FPC clinic directly by phone or via the patient portal to request a prescription renewal. We ask that you finish existing medication before requesting a new prescription. If you are overdue for a visit with your healthcare provider, you may be asked to schedule an appointment before we can renew your prescription.

We have healthcare providers on call at all times to assist our patients with acute medical needs when our offices are closed.

We send medical records by first-class mail via the U.S. Postal Service. We cannot fax or email medical records. We cannot expedite medical records requests. They are completed in the order in which they are received.
