FPC Patient Portal
The Family Practice Center, PC, Patient Portal provides secure access to portions of your medical record and allows easy and convenient communication with our practice. So that we can provide this service without compromising your private information, no one will have access to your personal portal account except you.

To start using the FPC patient portal, you will need to obtain an activation code from your FPC office. Please let us know if you need assistance.

Patient Portal Authorized Representatives
For Patients Who Are Minors: Due to federal and state confidentiality laws, patients who are between the ages of 14 and 17 are not required to disclose certain types of medical information to parents/legal guardians. This includes information regarding treatment for drug- and alcohol-related issues, mental health, reproductive health, and certain diseases. FPC is required to follow these laws.
For this reason, we find it necessary to ask our patients in this age group to complete a consent form naming a parent/guardian as their portal authorized representative. Without this consent from the minor patient, we will not be able to give parents/guardians access to the minor's portal. (Click the link below to access the form.)
Please note, this does not impact how you receive information and communicate with your child's health care providers outside of the portal. Parents/guardians may communicate with FPC providers and staff as you always have until your child reaches age 18.
For Adult Patients: Adult patients can grant portal access to an authorized representative such as a caregiver or an adult child who helps to monitor your health care. (Click the link below to access the form.)
Upon receipt of your completed/signed consent form, you will be given an activation code to begin using your Patient Portal Authorized Representative Account.
