1. Appointments?
Our offices are open from 7:00 a.m. to 8:00 p.m. Monday through Thursday; 8:00 a.m. through 5 p.m. Friday and Saturday; and Sunday as needed for acute problems only. Out of necessity for efficiency, we work by appointment. We urge that you be on time for your appointments. If you cannot keep your appointments, please notify us as soon as possible. We do accept walk-in patients, but will give priority to those who have scheduled appointments. When you come for your office visit, we request that you bring current medications you are taking. Please arrive 15 minutes before your scheduled appointment time so you can complete the necessary paperwork.

2. Telephone Calls?
Please call during regular office hours with non-emergency questions. As with any busy offices, our phones can be extremely busy. We apologize if this causes any inconvenience. Calls are coordinated by our clinical staff. They are trained to answer some of your questions. Others may need to be discussed with your doctor, and our staff will notify you of your physician's recommendations. To assist the physician, please be as specific as possible with your questions and messages. We will return phone calls after the morning and afternoon office sessions. On evenings, weekends and holidays, one of our physicians is always available. Call any of our office numbers and you will reach our answering service. The answering service can reach the physician on call with your message. Your call should be returned within 20 minutes. In addition, you may leave a message for the office staff for the next business day.

3. Prescription Refills?
Calls regarding prescription refills are handled by our clinical staff. We only authorize refills for medications prescribed by our physicians. It is our policy to prohibit prescribing medication over the phone for acute illness. Medication prescribed for chronic conditions may be refilled if the patient is up-to-date with their routine care for that problem. Refills for chronic medications should be directed to the office from 8:00 a.m. to 4:30 p.m. Monday through Friday. Please have the name and dosage information available. Prescriptions are called in by our clinical staff after our morning and afternoon office sessions are completed.

4. Hospitals?
Our physicians have active privileges in two hospitals in our area - Evangelical Community Hospital and Sunbury Community Hospital and Outpatient Center. One or more of our physicians are in the hospital every day. If you are hospitalized, one of our physicians will examine you daily. Before leaving the hospital, we will advise you as to medications and activities after discharge. We will be willing to answer any questions and also arrange for post-hospital office visits.

5. Obstetrical Care?
Although we do not deliver babies, we will take care of medical problems during pregnancy. Please do not take medications or treatments without consulting with your obstetrician or with us. We will see your newborn in the hospital and continue pediatric care at any of our locations.

6. Immunizations?
We do administer immunizations to our patients. The majority of insurance companies cover immunizations; however, it is your responsibility to know whether your insurance covers this service. We suggest you contact your insurance carrier to assure that you have such benefits. If you cannot afford the cost of immunizations, there are several government programs and agencies that can provide these services free of charge.

7. Laboratory and Radiology Services?
For your convenience, Family Practice Center offices have lab, x-ray, and EKG services on site. X-rays are interpreted by board certified radiologists, and results are forwarded to the ordering Family Practice Center physicians.

8. Work/School Excuses?
After being seen in the office for your problem, if you are in need of an excuse for work-school, we will gladly provide one with the information concerning your illness or injury.

9. Financial Policy and Insurance?
Reasonable fees are important when it comes to health care. We perform annual in-depth evaluations of our fees. We feel this fee schedule is reasonable and responsible. We cannot guarantee what part of services will be covered by insurance, but we will assist you in any way we can. However, you are ultimately responsible for your medical expenses and we will expect you pay on balances not covered by your insurance. Patients are requested to bring their insurance cards to their initial visit. As a courtesy to our patients, we will submit a claim to your insurance carrier for services performed at our facilities, provided you have given us all of the necessary information. Co-payments, deductibles, and non-covered charges are payable at the time of service. Please come to the office prepared to pay a portion of your charges. For your convenience, we accept payment by cash, check, MasterCard, and Visa.

We participate with the following insurance carriers: Blue Shield, Capital Blue Cross, Medicare, Senior Blue, Medical Assistance, Keystone Health Plan Central, Geisinger Health Plan, Health America, Susquehanna Health Care and Champus.

Please discuss any questions regarding financial difficulties with our staff early in your treatment so that appropriate payment arrangements can be made on your behalf.

10. Release of medical Information?
Our staff has the duty of confidentiality to the patient with regard to their medical records. The release of any information obtained within the course of the patient-physician relationship is prohibited without a signed medical release from the patient or his guardian.
