PATIENT PORTAL AUTHORIZED REPRESENTATIVE ACCOUNTS:
For Minor Patients: Due to federal and state confidentiality laws, minor patients, ages 14 to 17, are not required to disclose certain types of medical information to parents/legal guardians. This includes information regarding treatment for drug & alcohol related issues, mental health, reproductive health and certain diseases. FPC is required to follow these laws.
For this reason, we find it necessary to ask our patients in this age group to complete a consent form naming a parent / guardian as their portal authorized representative. Without this consent from the minor patient, we will not be able to give parents/guardians access to the minor's portal. (Click link below to access form.)
Please note, this does not impact how you receive information and communicate with your child's healthcare providers outside of the portal. Parents / guardians may communicate with FPC providers and staff as you always have until your child reaches age 18.
For Adult Patients: Adult patients can grant portal access to an authorized representative such as a caregiver or an adult child who helps to monitor your healthcare. (Click link below to access form.)
